In conjunction with our EHR provider CureMD, we offer limited secure access to select patient information. At this time, we offer access to demographic and lab results information as well as the ability to manage your portal access password.
We have recently upgraded our Patient Portal Access system to include broader and more robust security features for your health information protection. All existing Patient Portal accounts were deactivated.
Our new system includes more stringent user registration guidelines. Even though you may have been previously registered under our old system, we require your acceptance of our updated Terms & Conditions in order for your account to be re-activated. Below is a link to a secure form where you may update your information in order to re-activate your Patient Portal access.

If you no longer want to use your deactivated Patient Portal account, you need not take any action.
New and existing patients are offered the opportunity to complete a Portal Access Application during their visit. If your patient packet does not contain and application, please request one from the front desk and they will be happy to assist you.
